Helt, III, 43, of Worthington was convicted on a felony charge of aid and abet check forgery-offer/possess with intent to defraud; fined $630, restitution $545, commit to Commissioner of Corrections 17 months, stayed for five years, sentenced to jail 41 days, credit for time served 41 days, five years supervised probation, contact probation officer as directed, pay restitution of $545, pay restitution before fees, fines and surcharges, cognitive skill training if recommended by agent, complete and pay for a comprehensive assessment within 60 days of sentencing and follow all recommendations timely, must sign all releases for assessor to obtain collateral information from assigned probation agent(s), assigned social worker(s) if any, mental health treatment provider(s) if any, relevant family/household members and recent treatment providers as part of their assessment, must successfully complete the level of treatment and aftercare recommended rather than choosing a lesser level of care, no alcohol/controlled substance use with the exception of prescribed medications, no alcohol/controlled substance possession with the exception of prescribed medications, random testing, obtain employment, maintain employment, obtain permission from agent before leaving the state, follow all state and federal criminal laws, contact probation agent within 72 hours if charged with a new crime; are in contact with law enforcement; if changes address, employment, or telephone number, cooperate with search of person, residence, vehicle, workplace, property, etc. by probation officer, give a DNA sample when directed, sign releases of information as directed, don't use/possess firearms, ammunition, or explosives, don't vote until July 1, 2023, you have the right to register to vote on that date and any time you are not incarcerated for this offense.
